Desmopressin acetate, with a relative molecular mass of 1129.2, is a synthetic analogue of antidiuretic hormone (ADH) that can be used to treat nocturnal polyuria, diabetes insipidus, and control bleeding. Its antidiuretic activity, similar to endogenous vasopressin, primarily acts on the renal collecting ducts and the thick ascending limb of the medullary tubule. Its vasoconstrictive effect is much lower than natural vasopressin, approximately 1/400, and does not interfere with normal hormone production. Currently, its common clinical formulations include injection and oral tablets.


Desmopressin acetate is a synthetic analogue of antidiuretic hormone (ADH). This means it is a synthetic drug that mimics the action of natural vasopressin but specifically targets the V2 receptor. Therefore, it can be classified into two types:
(1) Antidiuretic: As mentioned above, it reduces urine output.
(2) Vasopressin analogue: Its action is similar to that of the natural hormone vasopressin.
Desmopressin acetate acts by mimicking the action of vasopressin, particularly targeting the V2 receptors in the kidneys. In the absence of endogenous vasopressin, desmopressin acetate binds to these V2 receptors, triggering cellular responses. This response increases the level of a molecule called cAMP inside kidney cells. The increase in cAMP then prompts the insertion of aquaporins, proteins responsible for water reabsorption, into the cell membrane. With more aquaporins available, the kidneys reabsorb more water from the filtrate, leading to a decrease in urine volume and more concentrated urine. This effectively conserves water in the body and helps maintain proper fluid balance.
In clinical practice, desmopressin acetate is commonly used to reduce bleeding time in invasive or diagnostic procedures and to treat patients with bleeding disorders due to uremia, cirrhosis, inherited or drug-induced platelet function abnormalities, or prolonged bleeding time of unknown cause, to restore normal bleeding time. Additionally, the medication can treat central diabetes insipidus. On March 3, 2017, the U.S. Food and Drug Administration (FDA) approved desmopressin acetate for the treatment of adult nocturnal polyuria.
For initial treatment, the recommended starting dose is 2 to 4 micrograms, administered subcutaneously or intravenously once or twice daily. The morning and evening doses should be adjusted separately to accommodate the appropriate diurnal rhythm of fluid turnover. The response to treatment should be adjusted based on two parameters: adequate sleep time and adequate but not excessive fluid turnover.
For patients previously treated with intranasal desmopressin, the recommended starting dose of desmopressin acetate injection is one-tenth of the daily maintenance intranasal dose, administered subcutaneously or intravenously once or twice daily.
The recommended dose is 0.3 mcg/kg actual body weight (up to a maximum of 20 mcg), administered by intravenous infusion over 15 to 30 minutes. If used preoperatively, administer 30 minutes before surgery. If used to reduce spontaneous or traumatic bleeding, repeat the dose every 8 to 12 hours based on clinical status and factor VIII levels, with a single dose per day thereafter. The need for repeat dosing of desmopressin acetate or use of any blood products for hemostasis should be determined based on laboratory response and the patient's clinical condition.
Headache, nausea, stomach discomfort, or facial flushing may occur. If these effects persist or worsen, inform your doctor or pharmacist promptly. Remember, this medication has been prescribed by your doctor because they have judged that the benefit to you is greater than the risk of side effects. Many people using this medication do not have serious side effects.
Severe allergic reactions to this drug are rare. However, seek immediate medical attention if you notice any symptoms of a serious allergic reaction, including: rash, itching/swelling (especially of the face/tongue/throat), severe dizziness, trouble breathing

If you are dehydrated, your doctor will correct this condition before starting desmopressin acetate treatment.
Limit alcoholic beverages, as they may interfere with the effects of desmopressin acetate.
If you experience any conditions that may lead to fluid/mineral imbalance (including fever, diarrhea, vomiting, flu-like infections), or if you encounter situations requiring increased fluid intake (such as exposure to very hot weather, vigorous exercise, heavy sweating), inform your doctor immediately. Your doctor may need to stop or adjust desmopressin acetate treatment, especially in children and the elderly.
